Transitional Housing Placement Program (THPP)

S.T.A.R. Programs includes a Transitional Housing Placement Program (T.H.P.P.) for ages 16-17 aimed at helping youth develop the independent living skills necessary once they emancipate from the dependency or juvenile justice system.


All participants receive case management services provided by skilled and trained professionals. 

Case Management Services:

  •  Placement decisions will not be determined by the youth's sex assigned at birth. All placement decisions for youth in care will be gender affirming and consider the safety, well-being, and voice of the youth.   
  • Appraisal needs and services plan development in collaboration with the each resident and their Social Worker/Probation Officer, based on their current Transitional Independent Living Plan (TILP).
  • Regular case management meetings to monitor needs, services and progress on defined goals.
  • Referrals to community partners to meet identified needs.
  • Frequent contact with Social Workers/Probation Officers, school personnel, therapeutic service providers and family members or other natural support persons as appropriate.
  • Quarterly progress reports on planned objectives and goals.

Highlights of T.H.P.P.

  • 14 bed facility serving male and female youth who are part of the child welfare or juvenile justice system ages 16-17.
  • Increased independent lifestyle based on demonstrated level of responsibility.
  • Supervision by trained qualified counselors with a Bachelor’s Degree or Associate’s Degree with equivalent experience.
  • Program builds a Savings Fund for each participant that is given to them at the time of emancipation or reunification.
  • Program emphasizes teaching youth independent living skills such as:

                           School and education planning
                          Work and career exploration
                          Money management and shopping
                          Housekeeping
                          Meal preparation
                          Transportation
                          Community resources
                          Sexuality/reproductive health
                          Time management and self-care
                          Securing post-emancipation housing


  • Job development skills such as resume writing, interview skills, job search strategies, networking, job shadowing and professional appearance/etiquette.
  • Required attendance at Independent Living Program (ILP) workshops following a referral from the authorized representative.

Transitional Housing Placement (THP-NMD)

S.T.A.R. Programs includes a Transitional Housing Placement Plus Foster Care (THP-NMD) for ages 18-21 aimed at helping young adults develop the independent living skills necessary once they exit the dependency or juvenile justice system.

All participants receive case management services provided by skilled and trained professionals. 

Case Management Services:

  •  Placement decisions will not be determined by the non-minor dependent’s sex assigned at birth. All placement decisions for youth in care will be gender affirming and consider the safety, well-being, and voice of the youth.   
  • Appraisal needs and services plan development in collaboration with the each resident and their Social Worker/Probation Officer, based on their current Transitional Independent Living Plan (TILP).

  • 7 bed duplex complex serving male young adults ages 18-up to 21. and two 6 bed homes serving male young adults ages 18-21
  • Increased independent lifestyle based on demonstrated level of responsibility.
  • Supervision by trained qualified counselors with a Bachelor’s Degree or Associate’s Degree with equivalent experience.
